***REMINDER to Parent/Guardian of Incoming 7th Grade Students: The Massachusetts Department of Public Health and Millis Public Schools require that all children have a current physical examination and are up to date with their immunizations before entering 7th grade. Please note the required MDPH immunizations your child must have on record before entering grade 7 include: 2 doses of MMR vaccine 3 doses of the Hepatitis B vaccine 1 dose of Tdap - usually given at the 10-12 year old well visit - please check to see that this has been given. This is required for the 7th grade and should be on most recent physical: 2 Varicella vaccines or documented Chicken Pox disease. Please send in documentation of the most recent physical exam including immunizations to the School Nurse before the end of this school year/summer if your child had a physical last winter or spring. If your child is due this summer or fall, please send the form in once the physical is done. A current PE is required if it has been 13 months since you have provided the school nurse with a copy of your child's physical exam. *Any form your physician fills out is acceptable. If your child will have an annual appointment scheduled for the Fall or if you have any questions, please let me know. Have a happy and healthy summer!
