Physical Exam & Immunization Requirements

The Millis Public Schools , in compliance with the Department of Public Health, require the following:  

 A copy of a physical exam and a current immunization record.  

The school nurse must receive these documents as follows:

* Entry to Millis Public Schools at any level

* Grades Pre kindergarten, Kindergarten, 1, 4, 7 and 10

* Annually prior to participation in competitive athletics
