FALL 2016 ATHLETICS and BAND REGISTRATION is OPEN!
**DEADLINES for ONLINE REGISTRATION AND SUBMISSION OF ANY REQUIRED PAPERWORK (Physicals, Concussion education certificates, and PREVIOUSLY OUTSTANDING USER FEES):
HIGH SCHOOL FOOTBALL (grades 9-12) DEADLINE: FRIDAY AUGUST 12th
ALL OTHER ATHLETES & BAND MEMBERS: THURSDAY AUGUST 18th
-NOTE: BAND members: online registration and concussion education ONLY- no physical exam form required unless also playing a sport. You will be able to select band in addition to a sport if necessary when registering. Please note there is NO FEE for band.

****ATHLETES WITH A PREVIOUSLY OUTSTANDING USER FEE BALANCE WILL NOT BE ABLE TO PARTICIPATE UNTIL BALANCE IS PAID IN FULL.****

***Athletes with outstanding FALL user fees after September 2nd will be prohibited from continuing participation until the user fee is submitted OR arrangements are made with the Athletics Department.***
$220 per athlete (no fee for band), payable by check or money order, to MILLIS PUBLIC SCHOOLS

THE FOLLOWING REQUIREMENTS MUST BE MET IN ORDER TO BE ELIGIBLE TO PARTICIPATE:
1) ONLINE REGISTRATION: Complete Fall 2016 sports/band registration on FamilyID. ***Registration is not complete until you receive your e-mail confirmation from FamilyID***
Fall Sports for students in grades 7-12 include:
Boys and Girls Soccer
Boys and Girls Cross Country
For registration purposes, just the specific sport needs to be indicated in the "Sections" portion. The level of an athlete's participation (i.e. varsity, JV, middle school, or freshman as applicable by sport) will be determined by coaches when sports begin.
BAND: Please remember to select BAND in addition to the athlete’s sport if he or she will be participating in both.
2) PHYSICAL EXAM: ***PHYSICAL EXAMS EXPIRE 13 MONTHS FROM THE DATE THE EXAM WAS ADMINISTERED. A copy of the athlete's current physical exam must be on file with the Athletics Department. Please submit a copy if you have not done so already.
**Should the physical exam expire during the season, the athlete will be ineligible to participate until a new current physical exam form is submitted to the Athletic Department. NO EXCEPTIONS will be made. PLEASE be aware of when it expires and make appointments well in advance.
PLEASE NOTE: If the physical exam date entered in the online registration does not match the date on the actual physical exam form provided to the school, eligibility may be affected!
3) ANNUAL CONCUSSION EDUCATION:
EACH athlete/band member AND a parent or guardian MUST complete the online concussion course AND submit a completion certificate to the Athletics Department ANNUALLY, per state law.
**A COMPLETION CERTIFICATE FOR ONE OF THE ONLINE COURSES MUST BE SUBMITTED IF IT HAS BEEN A YEAR OR MORE SINCE LAST SUBMITTED (i.e. Fall 2016 or earlier).
If you are not sure when one was last submitted, please submit a new one.
ONLINE COURSE options: ***EITHER COURSE IS ACCEPTABLE! Both are free.

In the registration process you will also be required to sign-off indicating that both a parent and each athlete have completed the necessary concussion education.
4) ATTEND A PRESEASON HEALTH AND SAFETY MEETING (Mandated by the MIAA)

ALL OTHER SPORTS: THURSDAY AUGUST 18th, 6:30-7:30pm

LOCATION: Football field bleachers; rain location TBD
5) BASELINE ImPACT CONCUSSION TESTING: This is different and totally separate from the state-mandated concussion education!!
**Testing will be scheduled once athletes have registered for their upcoming sport. Anticipated dates week of 8/22 and 8/29.
** WHO NEEDS TO TAKE A BASELINE TEST??
a) ALL ATHLETES who have not had a baseline in two or more years. This includes all new players, as well as ANY ATHLETE WHOSE LAST BASELINE WAS DONE IN THE FALL OF 2014 or earlier.
b) Players who suffered a concussion since their last baseline test. Post-injury ImPACT testing that was done is not the same as a new baseline.
Otherwise, baseline tests are good for 2 years.
